What can Hospice do for me?

Silver State Hospice Care works closely with patients’ attending physicians, hospitals, nursing homes, assisted living centers, insurers and community-based organizations to enhance our ability to address the discomfort, fears and high expenses that often accompany an incurable illness

This collaborative approach helps to identify and meet the specific needs of the terminally ill and their families. 

Many individuals are afraid of receiving Hospice Care due to incorrect perceptions. Silver State Hospice Care provides consultation and will take the time to sit down 

with you and your family and explain what Hospice is and what it is not.  Hospice is about the patient and the family. Hospice is able to provide many additional services that will provide needed comfort.

Flexibility--What is best for the patient?

Silver State Hospice Care provides care in the patients’ home or residence.  We also have close relationships with nursing homes and assisted living communities—wherever people could benefit from hospice care. The goal is to support patients and manage their care in the setting they consider home. If at some point a patient needs more constant care, Silver State Hospice Care  can provide 24 hour hospice care, for a brief period, to help the patient at home. If a patient needs care beyond what can be safely provided at home,  Silver State Hospice Care has special arrangements with local rehab facilities,

nursing homes and other facilities where patients may receive necessary inpatient care to manage the crisis until they can once again return home.

Hospice is covered 100% by Medicare, Medicaid and most private insurance. Since Silver State Hospice Care accepts Medicare and Medicaid as coverage for hospice care, there are no out-of-pocket expenses for eligible patients or their loved ones.

Our Silver State Hospice Care Team

 

ATTENDING PHYSICIAN

  • Certifies  the patient has a terminal illness and remains the patient's Primary Physician and works as a member of the hospice team

MEDICAL DIRECTOR

  • Certifies the Attending Physician's prognosis, leads the hospice team in developing a plan of care, provides consultation to the Attending Physician

REGISTERED NURSE

  • Evaluates the patient's condition during regular schedules visits. The Registered Nurse coordinates the plan of care and teaches the family proper patient care techniques and the use of medication and pain management

MEDICAL SOCIAL WORKER

  • Assesses social needs of the patient and family, offers counseling and as resources for community service

CHAPLAIN

  • Provides spiritual support and initiates contact with patient and family to determine their needs

HOME HEALTH AIDE

  • Provides direct personal care, ADL's and emotional support to the patient and family
