ATTENDING PHYSICIAN
- Certifies the patient has a terminal illness and remains the patient's Primary Physician and works as a member of the hospice team
MEDICAL DIRECTOR
- Certifies the Attending Physician's prognosis, leads the hospice team in developing a plan of care, provides consultation to the Attending Physician
REGISTERED NURSE
- Evaluates the patient's condition during regular schedules visits. The Registered Nurse coordinates the plan of care and teaches the family proper patient care techniques and the use of medication and pain management
MEDICAL SOCIAL WORKER
- Assesses social needs of the patient and family, offers counseling and as resources for community service
CHAPLAIN
- Provides spiritual support and initiates contact with patient and family to determine their needs
HOME HEALTH AIDE
- Provides direct personal care, ADL's and emotional support to the patient and family
